Safety critical systems are those systems whose failure could cause loss of life, significant property damage, or damage to the environment. 

Preliminary hazard analysis (PHA) is performed to identify potential hazards that could cause an accident. Hazards are categorized according to their severity.

Failure Mode Effects and Criticality Analysis
